Inside the game
The asylum theme surrounds a precise set of reel and persistence rules. The diagrams below show what each named mechanic actually changes.
Mental has five reels, a default 108 ways and up to 20 rows through its feature states. Ways and height change with the configuration; 20 rows is not the starting board.
Between one and 13 base-game positions can receive Fire Frames. A symbol inside a frame splits into two parts.
Two Dead Patient symbols activate a reveal of a Patient type and a value of 5, 10, 15, 30, 50, 100, 200, 500, 1,000 or 9,999. That value represents the number of that Patient symbol on the reel; it is not a direct bet-multiplier payout.
Enhancer Cells sit below reels 2 and 4. Four Fire Frames activate the reel-2 cell; six activate the reel-4 cell.
An active cell can reveal a Patient, Dead Patient, xWays, xSplit, xNudge Wild or regular Wild. These outcomes change different parts of the result.
xWays reveals three matching symbols and expands the reel. Multiple xWays reveal the same type; Scorpion, Spider and Dead Patient are excluded from that reveal. xSplit doubles one position on each reel and becomes two Wilds itself. xNudge moves a stacked Wild fully into view, adding one to its Wild multiplier per nudge.

A Spider on reel 2 or 4 triggers Mental Transform. It can turn into xWays, xSplit, Dead Patient, Wild or a Patient symbol.
In Lobotomy and Mental Free Spins, sticky Spiders repeat this transformation on each spin. The sticky state preserves the source of the effect, rather than guaranteeing the same transformation outcome.

Three Scorpions on reels 1, 3 and 5 start eight Autopsy Free Spins. Each triggering Scorpion in a Fire Frame adds one spin.
Up to five new Fire Frames can appear per Free Spin and remain for the feature. Spiders can upgrade the remaining spins into Lobotomy or Mental.

Three Scorpions and one Spider in the base game start nine Lobotomy Free Spins. One Spider during Autopsy upgrades the remaining spins and adds one. Triggering Scorpions or Spiders inside Fire Frames add a spin each.
The Spider stays and triggers Mental Transform every spin. Dead Multiplier records store the highest revealed value for each Patient type together with a Count of its reveals.
A stored value transfers to an eligible later Patient of that type, reducing its Count. Patient symbols created by Dead Patient do not receive that transfer.

Three Scorpions and two Spiders start ten Mental Free Spins. Two Spiders during Autopsy upgrade the remaining spins and add two; a new Spider during Lobotomy upgrades them and adds one. Fire Frame entry additions still apply.
Both Spiders become sticky. The decisive change is that Dead Multiplier Counts no longer decrease when values transfer, while further reveals can continue to build them.

The saved rules give 96.08% normal-play RTP. Evolution also lists a separate 94.20% provider build; do not combine that build with the paid-mode returns below.
Nolimit Bonus offers direct entries at 80×, 230× and 1,000× base bet, plus a 232× Lucky Draw. The saved €1 menu shows those same amounts in euros. The maximum base bet stated for feature purchases is €70.
The round maximum is 66,666× bet. Reaching the limit ends the round, including remaining feature activity.
The rules give a simulated maximum-payout probability of approximately one in 18 million rounds. That is a simulation measure for the top payout, not a general win or bonus frequency.
Wilds substitute for ordinary paying symbols but not Scorpions, Spiders or Dead Patients. Adjacent-reel wins run from the leftmost reel.
Read Patient values together with their reel-count role. A large Dead Patient number increases symbol multiplicity; a qualifying ways result is still needed for a payout.

The rules illustrate three Patient-1 reveals with values 5, 10 and 15. The stored value becomes the highest, 15, and the Count becomes three.
In Lobotomy the next three eligible Patient-1 appearances can receive that value, consuming one Count each. In Mental the Count no longer decreases on transfer.

Fire Frames do two jobs: they split symbols at their positions and contribute to the four/six thresholds for the Enhancer Cells.
During Free Spins, newly added frames stay for the feature. This lets later spins work with a developed frame pattern instead of starting the grid from scratch.

An xNudge Wild gains one multiplier step for each nudge needed to become fully visible. When several Wild multipliers apply, their values are added.
This Wild multiplier is separate from a Dead Patient symbol-count value and from the game’s total payout cap.
A split Scorpion or Spider is one trigger symbol; its Fire Frame can separately add a starting spin.
Scorpions do not appear in Free Spins, and Spiders do not appear in Mental Free Spins. Upgrades and entry additions have their own rules.
The 9,999 Dead Patient value is a symbol-count effect; it is not an automatic 9,999× award.
The four menu options have separate costs and theoretical returns. Lucky Draw selects an entry level rather than guaranteeing Mental Free Spins.
The saved menu shows a 50% Autopsy, 40% Lobotomy and 10% Mental selection split for Lucky Draw. These are entry-selection probabilities, not probabilities of winning a particular amount.

Mental suits players who enjoy learning how one feature changes another. The visible Fire Frame thresholds and three linked bonuses reward attention to the board state.
The cost is complexity: Patient values, Counts and Wild multipliers need separate interpretation. Players who prefer a single, immediately readable bonus may find the layered rules demanding.
Remaining spins continue with the sticky Fire Frame state and a new sticky Spider. Dead Multiplier tracking becomes part of the stronger feature.
The second sticky Spider is added and the Dead Multiplier state carries forward. Counts stop decreasing on transfer, changing how often a stored value can remain useful.
Each Patient type has its own stored value and Count. One Patient’s record is not a global multiplier for all five symbols.
The example below follows the rules’ Patient-1 illustration; it is not a recorded spin or a prediction.

A sticky Spider repeatedly opens access to the transformation outcomes: xWays, xSplit, Dead Patient, Wild or Patient. It does not mean every one of those effects occurs on every spin.
The difference from a newly landed base-game Spider is persistence: the source remains for later Free Spins.
The reel-2 cell activates at four Fire Frames, before the reel-4 cell at six. Track these separately when reading the bottom of the board.
Frames therefore affect both symbol density and access to additional symbol types. A new frame can matter even when its immediate split does not create a paying combination.

Splits one position per reel and turns itself into two Wilds. A new split copy does not inherit an existing Dead Multiplier.
Moves a Wild fully into view and grows its Wild multiplier by nudge count.

Begin with the frame positions and the two Enhancer counters. Then identify which effect was revealed before interpreting the changed reel height or multiplier.
In Lobotomy, watch a Patient’s Count decrease when its stored value transfers. In Mental, the same transfer leaves the Count available. That contrast explains the top bonus more clearly than its name alone.
The largest figures on screen describe different things. A 9,999 Patient value changes symbol multiplicity. The 66,666× figure is the round payout limit. One in 18 million is the stated simulated probability of that limit.
None of these establishes how often an ordinary win or a Free Spins trigger occurs.
Use the game’s normal-play RTP when comparing its base configuration. The saved version states 96.08%; the provider also lists a 94.20% build.
The 96.26%, 96.17%, 96.68% and 96.42% figures belong to the separate Autopsy, Lobotomy, Mental and Lucky Draw purchases. A higher listed buy RTP does not remove the much larger amount risked at entry.
The stronger Free Spins modes retain the sticky Fire Frame mechanic alongside the added Spider state.
The rules exclude Patient symbols revealed through Dead Patient from the normal stored-value transfer.

Mental’s complexity has a structure: Fire Frames develop the board, Spiders connect to several reel effects, and the bonuses change what remains between spins. The diagrams make those connections more useful than a flat list of branded features.
The most consequential upgrade is Lobotomy to Mental. Both use stored Patient values, but Mental stops consuming their Counts on transfer. That is a deeper change than simply adding another starting spin.
The numerical detail also needs care. Patient values are not direct cash awards, and the simulated one-in-18-million maximum-payout probability says nothing about ordinary hit frequency. The game fits players who want a layered system, with a substantial learning curve and high provider-rated volatility.
